Remediation Comments

Please submit one form per student per class. If you are submitting remediation comments for the same student in LS and in RW, submit two forms.

Student Information

Please enter the student's name as it appears on your roster. If you want to include a nickname that the student prefers, place it in the "Student's First Name" field after their official first name and surrounded by parentheses or quotation marks: e.g. Jennifer "Love" or Jennifer (Love).

Student's UDID (Please do not leave this blank. You can see UDIDs on your class roster. Incorrect ID numbers will cause reporting problems.)

Classes

Please choose the class for which you are submitting this student's remediation information. Do not enter classes in the "other" option of the dropdown boxes. E.g., if you are submitting remediation comments for this student's performance in your RW class, choose your class from the "Reading/Writing Class (includes ARSC/ENGL)" dropdown box. Please do not enter the student's other classes in the "other" field option of the dropdown boxes.

If you have the same student in LS and RW and need to submit remediation comments for the student in each of their classes, please submit two forms, one for each class.

Reading/Writing Class (includes ARSC/ENGL)

AT Content Class (includes AT Cohort and UNIV101)
